Human failure types

Explosions, road accidents and other unexpected incidents are often said to be caused by ‘human error’. However, this is really no explanation at all, and it tells us little about the real causes of an incident. The Energy Institute’s Stuart King, Hearts and Minds Programme Officer, explains.
